“Your Voice, Your Choice” Advance Care Planning Workshop

Join us for an interactive workshop on completing an Advance Directive. All participants will be guided through this simple yet thought provoking process of choosing a health care agent and putting your wishes in writing related to your health care in the event that you are unable to speak for yourself.
